ACT Health have provided a consultation paper for the 2023 ACT Primary Care Pilot.

The stated aim of the model is “to strengthen partnerships between primary care, community based care and the public health system to support access and care for patients who frequently visit the ED or who are at risk of presentation to the ED or admission to hospital.”
