SWSLHD is proposing organisational changes in Community Facing Aged Care services.

The proposal involves the implementation of an integrated multidisciplinary model of care called the Community Older Person’s Intervention and Liaison Outreach Team (COPILOT) which will see the consolidation of siloed services as well as the relocation of the Community Outreach Geriatric Service (COGS) and changes to reporting lines.
